Please download and complete our Patient Health History and Financial Policies forms and bring them to your first appointment. You can also arrive early to your first appointment and fill these forms out in our waiting area. If your paperwork is not complete before your appointment time, you may not be seen that day. Please refrain from wearing perfume, cologne or essential oil during your treatment.
