If you have lost or misplaced your original CEU certificate and need a copy to renew you license, please contact the Office of Continuing Education at 503-253-3443 x204. Please include all the following information in your request:

• Full name used at time of seminar attendance
• License number (for California LAc)
• Year of seminar
• Complete mailing address
• Day and evening telephone numbers
• E-mail address

There is a $10 processing fee. Make your check payable to OCOM and mail to: Oregon College of Oriental Medicine, Attn: Office of CE, 10525 SE Cherry Blossom Drive, Portland, OR 97216.

It will take approximately two weeks for OCOM to re-issue your CEU certificate. If you have more questions about CEUs, please contact the Office of Continuing Education at 503-253-3443 x204.
